Post-Grant Feedback Form
The Duxbury Education Foundation requests grant recipients submit the post-grant feedback form within two months of the grant's implementation to assist DEF in improving the grant process. Also, DEF requests at least one photograph of the grant in action to include in the Duxbury Education Foundation Annual Report, on the website and/or on social media sites. Photographs should be full-size jpeg files.
Before submitting photographs, please confirm that any pictures showing students are approved by the school to be used in DEF's social media and marketing materials.
